    Assessing independent living skills and exploring future housing and living arrangements that support long-term independence and stability

    Exploring vocational training, employment pathways, and postsecondary education opportunities

    Creating customized transition and long-term sustainability plans that align with the individual’s strengths, interests, and future goals, including planning for supports beyond the parents’ lifetime

    Networking and connecting families with potential team members including but not limited to Estate Planners, Trustees, Financial Advisors, Supports Coordinators, Evaluators, etc.

    Collaboration with trusted professionals to coordinate transition planning and long-term financial, legal, and support strategies

    Identifying adult service providers, community resources, and opportunities for meaningful engagement in the community

    Preparing siblings and future supporters for their evolving roles in supporting long-term planning and advocacy

    Creating contingency plans to address emergencies, unexpected changes, or future transitions in care and support
